Appointment Preparation

You will save a lot of time and energy if you take a few minutes to prepare for your first appointment at the Center for Women's Health.

Please bring the following items to your appointment:

• Your insurance card (see below for participating insurance companies)
• Your co-pay
• Completed new patient paperwork, if applicable (refer to sidebar link)
• Signed parental consent form, if applicable (refer to sidebar link)
• List of current medications, including strength and dosage
